the best way to do is make good content and quality Link placement with relevancy.
Try to build more links with required two keywords.
I have faced the same problem now i think its time to do more on page marketing rather then making content farms and other strategies.
Well there are alot of different software who can track your seo work but the best free way is to check through yahoo site explorer its better .
Well i agree with the above the formula of Page rank and SERP ranking but it has more then 200 factors.
